Introduction:
Intrauterine volvulus, an uncommon fetal disorder, poses diagnostic challenges during pregnancy. This article discusses a case of intrauterine volvulus, highlighting the need for early detection, prompt delivery, and urgent surgical intervention to ensure fetal and neonatal well-being.

Case Presentation:
A pregnant woman, gravida 4, para 1, with a complex medical history, presented with signs of preterm labor and reduced fetal movements in her 35th week. Ultrasonography revealed a markedly distended fetal bowel, raising suspicion of intrauterine volvulus. The baby was delivered via cesarean section due to deteriorating abdominal conditions. Urgent exploratory laparotomy was performed on the newborn, resulting in successful recovery and follow-up care.

Discussion:
Intrauterine volvulus, a rare condition, can be challenging to diagnose antenatally. Specific sonographic features, such as marked intestinal dilatation, suggest bowel obstruction and prompt the need for further investigation. Delayed diagnosis and intervention may lead to fetal or neonatal mortality.

Conclusions:
Early detection, close monitoring, prompt delivery, expert neonatal and pediatric surgical care, and nutritional support are essential for managing intrauterine volvulus. This article emphasizes the importance of careful fetal sonography, particularly in high-risk cases, to ensure the well-being of both mother and baby.
